Overview
CAPRI for Veterans Service Organizations (VSOs) is a One VA initiative which allows authorized VSO representatives read-only access to a specified claimant’s VHA electronic health record (EHR). VSO access to CAPRI is contingent upon the claimant executing a power of attorney (POA) authorizing the VSO unlimited and unrestricted access to all medical records, including sensitive records protected by title 38 U.S.C § 7332.
VSO CAPRI Access
The Special User Access Form for VSO is used by accredited VSOs and Private Attorneys (PA) to request access to CAPRI from National Data Systems (NDS) Health Information Access (HIA), in coordination with a VBA approver. The approver is a VBA employee associated with the VSO/PA’s Regional Office (RO) and is a Management Analyst (MA), or Change Management Agent (CMA). The CMA is now responsible for approving VSO access to CAPRI in place of the Information Security Officer (ISO). The VSO representative must be accredited by the Office of General Counsel prior to authorization for CAPRI access.
Instructions for Completing Special User Access Form
Section 1: To be completed by VSO Representative.
Open Special User Access Form for VSO.
Enter VSO information in boxes 1-12.
Sign as the Individual Requesting Access on page 1.
Sign the VSO Electronic Health Record Access Agreement on page 2.
Submit the form to your VBA Regional Office CMA. Do not send your form to the ISO, the process has changed.*
Section 2: To be completed by the VBA CMA (formerly completed by the ISO).
Enter VBA CMA information in box 15 & 16.
