It is important that we have all treatment facilities, especially civilian treatment facilities, that you have been to regarding the conditions/injuries you would like to file for.  We will be sending this information to the VA to pull medical records for evidence use. If you have any treatment facilities to add, please provide the facility name, the city/town of their location, and what year you first started treating there. Please use the format shown below:

This information is used in the event your claims are granted; we will then gather the appropriate information to add your dependents to your claims for increased compensation.

   • Housing Assistance - Are you homeless or at immediate risk of becoming homeless? Are you behind or soon to be behind on your rent? If so, I can forward your information over to our SSVF Department to see if you are eligible to receive housing assistance.
   • Employment Assistance - Are you looking for a new job or a job that better fits your needs? If so, I can forward your information over to our Employment & Training Department to see what opportunities we may be able to provide.

Once you have confirmed this information, I will send you a Representation Packet through a program called DocuSign. Docusign requires an e-signature that can be completed on any smartphone or computer. Please let me know if you have any questions.
