It is important that we have all treatment facilities, especially civilian treatment facilities, that you have been to regarding the conditions/injuries you would like to file for.  We will be sending this information to the VA to pull medical records for evidence use. If you have any treatment facilities to add, please provide the facility name, the city/town of their location, and what year you first started treating there. Please use the format shown below:
