Subject: Your VA Case: IMPORTANT INFORMATION!!
We have submitted a VA Form 21-4142 requesting that the VA obtain your private medical records from one or more medical providers outside the VA. The VA will send the following letter(s) in response to this Private Medical Request.
The VA may send 3 letters when attempting to gather private medical records.
- The first letter states that the VA has sent a request to the listed facilities to obtain your records. This is the VA’s first attempt at gathering these records. This letter does not require any action to be taken. – This is the letter you received.
- The second letter will state that the VA has tried once to obtain these medical records from the listed facilities but has been unable to do so. It will also state that the VA will make one more attempt to gather these medical records. This letter does not require any action to be taken.
- The third letter will state that the VA has “exhausted all available options” for obtaining the medical records from the listed facilities and now needs you to reach out to the medical providers for these records to be considered. This letter does require action. If you receive this letter, please contact the facilities listed to attempt to obtain your records. Please give the office a call to let us know that you received the letter stating that the VA has “exhausted all efforts to obtain the records”.
