Login to jotform and navigate to the My Forms page
Click on the Service Connection Form and Edit Form
Navigate to the Publish Tab at the top of the form
Select PREFILL from the tabs on the left-hand side
Select MANUAL PREFILL
Click + Add a New Prefill and fill in the fields in the form for the client
- MAKE SURE YOU PUT THE CLIENT’S NAME IN YOUR PREFILL
Once you have filled in the information you have, click + Create in the top right hand corner
Click the 3 vertical dots next to the prefilled form you just created for the client and select Copy URL or Send Invitation to email the client the unique link to the form that has been prefilled for them:
Make sure the client knows when the client opens the link, they must click Continue with Draft:
