I just sent you the Representation Packet with a link to esign the documents necessary to become your representative through docusign. These documents authorize our licensed disability attorney, Kailey L. Wildenhain, Esq., and our office, to represent you before the VA. You can esign on any smartphone or computer.

This packet contains the following documents:

1. Appointment of Representative (VA Form 21-22a)
   • This gives us access to your VA file, including any prior decisions, service records, medical records, and all evidence you’ve submitted in the past.
2. Fee Agreement
   • You will receive the full amount of your retroactive award for all claims granted on initial. This means if it is the first time you are filing for a claim, there will be no attorney fee withheld. Only when an appeal is granted will the VA withhold 20% of your retroactive award as an attorney fee.
3. Authorization to Release Certain Medical Records (VA Form 21-4142)
   • This gives the VA permission to request copies of any medical records from facilities outside the VA medical hospitals. While not everyone has treated outside the VA, we get this authorization just in case we need to request outside medical records as part of the development of your case.

Please read each document prior to signing. Please ensure your name, date of birth, social security number, and address are correct on all forms. This is the most important information for obtaining representation. If any information is incorrect, please let us know so we can correct this information as needed.
